New patients

Please note that we currently are not able to accomodate any new patients to the practice. Thank you for your interest, and please check again in a few months for updates.

Ontario Family Practice require all new patients to complete and submit the Patient Registration Form prior to their appointment.

Please arrive at least 15 minutes prior to your scheduled appointment time so that you can complete the required registration paperwork.

We offer routine appointments on a day to day basis; however we also keep openings for emergencies on a daily basis. Our top priority is treating children presenting with urgent medical needs.

Transfer of Medical Records Form

New patients will also need to transfer their medical records to this practice. To do this, complete the Transfer Medical Records form at the front desk and we will transfer medical records from your former practice.

Reminder System

Occasionally, our practice may send out a reminder notice to inform you of preventative health services appropriate to you.

SMS reminders are sent to you to ensure that you don’t miss your appointment. It’s possible to opt out of either the reminder notices or SMS notifications on request.

Repeat prescriptions

All requests for repeat prescriptions are reviewed by the doctor. A decision will be made about your eligibility for a repeat. The prescription will generally be available within 1 business day.

The receptionist will call to advise if your repeat prescription request has been declined, and to organise an appointment to see the doctor.

Results

Your doctor will advise you during the consultation when they expect your results to arrive at the practice. We have commenced a new results notification by SMS system from August 2020.

After all your results have been checked by your doctor, an SMS message will be sent out informing you about your results. Depending on the nature of your results the SMS will advise you if no action is necessary, or if you are required to book an urgent or a non-urgent follow-up appointment.

You will also be reminded by another message in a week if you have failed or forgotten to make the follow-up appointment as recommended.

Please note that no test results are filed in a patient’s computer file without being checked by the Doctor first. For patients who do not have a mobile phone, abnormal results will be conveyed by a phone call from the doctor or reception staff as necessary.

Please ensure that your mobile phone/ home phone contact details are up to date so that we can contact you with specific follow-up instructions in a timely manner.

Please keep your appointment if one is made as a follow-up during your consult. We will take all possible steps to ensure that all your test results are conveyed to you, but please notify us if you do not receive any notification about your results from us.

Ontario Family Practice treat your medical records with the utmost respect for privacy, security and confidentiality. All medical data is kept secure from unauthorised access, use or loss. It is only available to authorised members of staff.
